Since its beginnings in 2016, CBHM has grown to include community organizations, faith-based leaders, non-profits and social service agencies throughout Central Brooklyn and East New York. These partnerships have conducted outreach and health education throughout Central Brooklyn to encourage positive change.  

Together with support and involvement from local government, congressional district leaders, and other community leaders, YOU are key to ensure the success of CBHM.
